Why this happens
Notification routing sending to the wrong place
Gravity Forms can route notifications conditionally by field value. A new option added to a dropdown without a matching routing rule produces submissions that notify nobody.
The notification event is not 'Form is submitted'
Notifications tied to a payment or feed event never fire if that step is not reached.
From Email failing authentication
As with every WordPress form, sending as the visitor's address triggers SPF and DMARC rejection.
A feed erroring before notification
If a CRM or payment feed throws, later steps in the chain — including notifications — may never run.
