Why this happens

No SMTP configured

The Divi module uses wp_mail(), so it inherits every WordPress delivery problem — most often PHP mail() being blocked by the host.

The email address field left empty

If the module's own Email Address setting is blank, Divi falls back to the site admin email, which is often an address nobody reads.

The basic captcha rejecting real visitors

Divi's built-in arithmetic captcha is easily mistyped on mobile, and a failed attempt is not always obvious to the visitor.

A cached page serving a stale nonce

Aggressive page caching can serve an expired security token, so the submission is rejected before it is processed.
