Notification recipients not set on the form
Each form has its own recipient list under Options. A new form created by duplicating another does not always carry it across.
A workflow handling notifications that is paused
Where notifications are sent by workflow rather than by the form, pausing or unpublishing that workflow stops them with no change to the form.
The embed script blocked before it loads
Consent management tools frequently classify the HubSpot embed as non-essential and block it until the visitor accepts. Visitors who decline never see a form at all.
Progressive fields or GDPR options changing the form
